ACTION REQUIRED- FOP INSURANCE CENSUS

URGENT: Action Required – FOP Insurance Census

Members and Nonmembers,

We need immediate participation in the FOP Insurance Census. This step is mandatory for every employee covered under Unit I and Unit N — including non‑members. Without complete census data, insurance carriers will not provide quotes or enrollment options, and we cannot move forward in securing improved coverage.

We are restarting the process, so everyone must complete this again, even if you submitted the previous survey. Only the information required by insurers is being collected, and it will be used exclusively for obtaining insurance quotes and enrollment.

Please complete the census as soon as possible using the link or by scanning the QR code with your phone’s camera.

If we want better insurance options, full participation is essential. We cannot negotiate improved coverage without everyone’s cooperation.
