All COT’s need to read this message and make sure they are submitting the LRU-5 form at every stage of the hiring bonus. This is how the state ensures that they are properly compensated. Please ensure that any Trainee who was hired on or after July 1st, 2023 fills out this form. All COT’s you will need to forward this form to your immediate supervisor, Hope Chowanski- hechowanski@doc.nv.gov, and your local payroll administrator. Make sure you do this for each step of the hiring bonus, do not wait for your supervisor to request it from you. If you have missed portions of the hiring bonus, be sure to submit one for each portion that you have not received.
